If you or your loved one is recovering from an injury or recent illness, skilled medical care may be necessary. Our goal at Michigan Community VNA is to keep patients at home and out of the hospital. Our Registered Nurses evaluate, oversee and coordinate the care your doctor recommends. Our specially-trained nurses partner with you and your family or other caregivers. We can help improve conditions or prevent a current condition from worsening – so your loved one can feel better, function more independently and learn to take charge of his or her health needs.
We tailor our services and education to you whether it’s for managing heart disease(Hearts At Home Program), diabetes or other chronic condition, tending to a wound, managing pain, addressing ostomy/continence issues or treating lymphedema. Our goal is to help you recover from an illness or injury, recuperate after surgery, and get the tools and information you need to manage chronic conditions and live well, and independently.
