Visiting Nurse Association of Southeast Michigan understands that your health
care information is personal and confidential. We are committed to protecting
this information. We create a medical record of services that we provide to
you. We use this record in order to provide you with quality care and to comply
with certain legal requirements. This notice applies to all the records of your
care that we create. Your personal doctor may have different policies or
notices regarding his/her use of and disclosure of your health care
information.
This notice will tell you about the ways in which we may use and disclose health care information about you.
We also describe your rights and certain obligations we have regarding the use and disclosure of health care information.
