Hospice is a service that provides professional, compassionate care for people at the end of life and their families.
VNA's team approach can include
your physician, family members and caregivers. Under the direction
of VNA's hospice physician, our experienced team of nurses, social
workers, spiritual counselors, therapists, home health aides and
volunteers works closely with you and your family to meet your
physical, emotional, spiritual and bereavement needs.
While services are provided primarily in a person's home with respect for ethnic and cultural diversity, hospice care can be provided anywhere including assisted living facilities and hospitals.

Hospice
Philosophy
Hospice affirms life. Hospice exists to provide support and comfort
to dying patients and their families in the terminal phase of
illness. Hospice supports these patients so that they might live
as comfortably and as fully as possible. Hospice neither hastens
nor postpones death. Hospice exists in the hope and belief that
through appropriate care and the promotion of a caring community,
patients and families may be free to attain a degree of mental
and spiritual preparation for death that is satisfactory to them.
Services
VNA Hospice care is individualized to your needs. An on-call nurse
is is available 24 hours a day, 7 days a week.

Eligibility
VNA of Southeast Michigan's hospice program serves:

Any
individual whose physician has prescribed hospice care who
is a resident of Wayne, Oakland or Macomb counties. |
| |
All adults,
regardless of ethnic background or religious affiliation.
Prior hospitalization is not required. |
Cost
VNA is certified by Medicare, Medicaid and Blue Cross Blue
Shield of Michigan as a provider of hospice care. VNA also accepts
other insurances.
Medicare Coverage
When a Medicare-eligible hospice
patient receives services from a Medicare-approved hospice, Medicare
will cover:
