Hospice is a program of care for individuals in their final stage of life, their families and their caregivers . VNA's hospice staff are experts at providing hospice care in a patient's home or in a home-like setting such as an assisted-living center, nursing home or hospital.

VNA's hospice program offers a team approach that can include your physician, family members and caregivers. Under the direction of the hospice physician, our experienced team of nurses, social workers, spiritual counselors, therapists, home health aides and volunteers work closely with you and your family to meet your physical, emotional social and spiritual needs.

Our Mission
VNA's hospice program is committed to providing caring and supportive services to terminally ill patients and their family. High-quality services are provided primarily in the home in a compassionate, dignified manner with respect for ethnic and cultural diversity. These services are provided in a way that ensures continuity of care among all hospice team members that addresses the individual's and family's physical, psychosocial and spiritual and bereavement needs.


Hospice Philosophy
Hospice affirms life. Hospice exists to provide support and comfort to dying patients and their families in the terminal phase of illness. Hospice supports these patients so that they might live as comfortably and as fully as possible. Hospice neither hastens nor postpones death. Hospice exists in the hope and belief that through appropriate care and the promotion of a caring community, patients and families may be free to attain a degree of mental and spiritual preparation for death that is satisfactory to them.



Services
VNA Hospice care is individualized to your needs. An on-call nurse is is available 24 hours a day, 7 days a week.

  • Specialized care
  • Physical, occupational and speech/language therapy
  • Nutrition counseling
  • Respite care
  • Companion and respite services by trained volunteers
  • Medications for pain relief and symptom management
  • Medical supplies and equipment
  • Up to 13 months of bereavement services

    Eligibility
    VNA of Southeast Michigan's hospice program serves:
  • Any individual whose physician has prescribed hospice care who is a resident of Wayne, Oakland or Macomb counties.
  • All adults, regardless of ethnic background or religious affiliation. Prior hospitalization is not required.


    Cost
    VNA is certified by Medicare, Medicaid and Blue Cross Blue Shield of Michigan as a provider of hospice care. VNA also accepts other insurances. United Way for Southeastern Michigan funds may be available for patients without insurance.


    Medicare Coverage
    When a Medicare-eligible hospice patient receives services from a Medicare-approved hospice, Medicare will cover:
  • Physician services
  • Nursing care
  • Medical appliances and supplies as appropriate
  • Medication for symptom management and pain relief
  • Short-term inpatient and respite care
  • Home health aide
  • Physical and other therapies
  • Counseling
