VNA of Southeast Michigan is the
largest, independent nonprofit home health care and hospice
provider in southeast Michigan and makes more than 135,000
visits to patients annually. |

Our Medicare-certified home health
care and hospice services cover Wayne, Oakland, Macomb, Monroe,
Livingston and Washtenaw counties and surrounding areas. |

Our TeleHomecare program for heart
failure received international recognition from the Association
of Telehealth Services Providers. OUr program serves patients
with hear tfailure, hypertension, insulin-dependent diabetes
and COPD. |

VNA receives funding from United
Way for Southeastern Michigan to provide home health care
and hospice services to uninsured community members. City
of Detroit Neighborhood Opportunity Fund (NOF) monies are
available for Detroit residents. |

With Language Line certified medical
interpreting service, VNA staff members have 24-hour access
to interpreters who bridge the gap between patients who speak
little or no English and their health care providers. |

VNA addresses community needs
by providing blood pressure screenings, falls prevention screenings,
convenient access to meningitis and flu shots and more tha
$1 million in uncompensated home care services to the uninsured
and underinsured. |

Through an independent patient
satisfaction survey, 96% of our patients believed we met their
needs well, very well or excellently. |
